Why Bad Breath Isn’t Always About Oral Hygiene

Bad breath has a way of making people defensive.

Almost every patient who raises the issue starts the same way—by apologising. They assume they’ve done something wrong. Not brushing enough. Missing floss. Skipping something obvious.

In many cases, they haven’t.

In fact, some of the most persistent bad breath we see comes from patients who are already doing more than average when it comes to oral care. That’s usually when confusion sets in. If the hygiene is good, why hasn’t the problem gone away?

Because hygiene isn’t always the problem.

The Smell Usually Comes From Bacteria — Not Neglect :

Garlic and onions get blamed a lot. They’re obvious. But coffee, alcohol, smoking, and vaping are far more consistent offenders because they dry the mouth.

Certain diets can also change how the body metabolises energy, which affects breath chemistry. This isn’t common knowledge, and most patients don’t connect the two.

But the pattern is familiar in a clinical setting.

Sometimes the Mouth Isn’t the Source :

Occasionally, everything in the mouth checks out.

When that happens, we look beyond it. Sinuses. Tonsils. Reflux. Post-nasal drip. Medical causes do exist, and they’re often overlooked because the smell is assumed to be dental by default.

Ruling out oral causes first makes the next step clearer.

Why Mouthwash Rarely Solves the Problem :

Mouthwash is cosmetic.

It masks odour temporarily. It doesn’t remove the cause. In some cases, especially with alcohol-based rinses, it makes dry mouth worse and allows bacteria to rebound faster.

Fresh breath isn’t about stronger products. It’s about accurate diagnosis.
